Statistical Review of Gastric Surgery Cases


Abstract
The statistical review of our experience with 285 cases of gastric surgery case admitted to the
Department of Surgery, Chonnam University Medical College during the period from Mar. 1967 to
Feb. 1971 was done.
The results are as follows:
1) In 285 cases 72 cases are gastric ulce, 82 cases are duodenal ulcer, 8 cases are combined gastric
and duodenal ulcer, 7 cases are hemorrhagic gasritis and 112 cases are stomach ca.
2) The percentage ratio of gastric surgery cases to the total number of admitted cases (23,621 cases)
in the Department of Surgery during the above mentioned period was 1.07%.
3) The peak age incidence in gastric ulcer is 41¡­50 age group of 36.1%, in duodenal ulcer 31¡­40
age group of 42.8%, in combined gastric and duodenal ulcer 31¡­40age group of 37.5%, in gastric ca
41¡­50age group of 38.5%, and in hemorrhagic gastritis 31¡­40age group of 54.5%.
4) The ratio of male to female is dominantly high in male; 7:1 in gastric ulcer, 12:1 in duodenal
ulcer, 7:1 in gastric and duodenal ulcer, 22:1 in gastric ca and all male in hemorrhagic gastritis.
5) The subjective symptoms in order of frequency are as follows; indigestion 145 cases (89.5%),
epigastric pain and discomfort 136 cases (84%), anorexia 86 cases (53.0%) in peptic ulcer disease but
epigastric pain and discomfort 99 cases (88.3%), nausea and vomiting 68 cases (60.7%), epigastric
fullness 63 cases (56.2%), indigestion 62 cases (55.3%) in gastric ca.
6) The objectice signs in order of frequency are as follows; epigastric tenderness 104 cases (64.2%),
weight loss 67 cases (41.4%) in peptic ulcer disease but palpable mass 69 cases (61.6%), weigh loss
61 cases (54.5%), epigastric tenderness 33 cases (29.5%) in gastric ca.
7) The most frequent duration of symptoms are 3¡­5 years (41.3%) in peptic ulcer disease, 6 mon
ths¡­1 year (44.5%) in hemorrhagic gastrics, 3¡­6 months (26.7%) within 3 months (24.1%) in
gastric ca.
8) In laborartory finding, hemoglobin ranged from 10gm% to 15gm% is 54.3% in peptic ulcer
disease but 5gm% to 10gm% is 88.3% in gastric ca. The most common blood type is O type (38.2%)
in peptic ulcer disease but B type (35.1%)in gastric ca. Gastric analysis shows 51.9% normochlorhydria
and 33.3% hypocholorhydria in gastric ulcer, 89.2% hypochlorhydria in duodenal ulcer and 72.8%
acholorhydria in gastric ca. Appearance of occult blood is 66.6% in gastric ulcer, 64.3% in duodenal
ulcer and 74.8% in gastric ca.
9) Operative procedure are as follows; 50¡­60% distal gasrectomy 51 cases(70.8%), subtotal gast
rectomy 18 cases(25%) and gastroenterostomy 3 cases(4.2%) in gastric ulcer, and subtotal gastrectomy
60 cases(73.1%), vagotomy with antrectomy 17 cases(20.8%), gastroenterostomy 5 cases(6.1%) in
duodenal ulcer and vagotomy and pylotoplasty 5 cases(45.4%), subtotal gastrectomy 3 cases(27.3%),
vagotomy and antrectomy 3 cases(27.3%) in hemorrhagic gastrics, and subtotal gastrectomy 48 cases
(42.8%), radical subtotal gastrectomy 31 cases(27.7%), gastroenterostomy 8 cases(7.1%), radical
subtotal gastrectomy with surr(ucding suture manipuration 11 cases(9.9%), total gastrectomy 12 cases
(10.7%), esophagogastrectomy 2 cases(1.8%) in gastric ca.
10) The most frequent location of lesion are as follow; Pylorus was 80.5% in gastric ulcer, duodenal
first portion was 97.8% in duodenal ulcer and pylorus was 48.2% in gastric ca.
11) The operative complication are observed in 48 cases(16.9%), of which the most common com
plications is wound infection 15 cases (5.4%)
12) The cases of reoperation are 9 cases(3.18%)
13) The operative mortality is 1.67%(5 cases)
14) The causes of death are 2 cases of renal failure, 1?case of sepsis, 1 case of irreversible shock,
1 cases of cardiac failure.
